Who we send your way
The families we connect with skilled nursing partners
- Patients who need daily therapy after a hip, knee or spine surgery
- Older adults recovering from a stroke, sepsis or a long hospital stay
- Families whose loved one needs more medical care than assisted living can give
- Families planning a long-term stay and exploring Medicaid
- Families unhappy with a current facility and looking for another option
Families always choose their own provider. We introduce options; we don't make the decision for them.

Skilled Nursing partners: common questions
Do you work with hospital case managers?
Our focus is families. Many reach us while their loved one is still in the hospital, and we encourage them to share the facilities they choose with the hospital team.
Do you refer long-term Medicaid residents?
Yes, to facilities that tell us they have Medicaid beds. We help families understand that Medicaid has its own financial rules in every state.
Do you share star ratings with families?
We point every family to Medicare's Care Compare. We would rather you tell your story alongside your ratings than have families find them on their own.
