Who we send your way

The families we connect with home health partners

  • Patients heading home after joint replacement, cardiac or abdominal surgery
  • Older adults recovering from pneumonia, a fall or a stroke
  • Families managing a new diabetes, heart failure or COPD diagnosis at home
  • Patients finishing a skilled nursing stay who still need therapy
  • Long-distance children arranging care for a parent who lives alone

Families always choose their own provider. We introduce options; we don't make the decision for them.

Home Health partners: common questions

Do families arrive with a doctor's order?

Some do and some do not. We explain the order, homebound and face-to-face requirements so families know what their doctor or the hospital needs to provide, and we share what we know with you when we connect you.

Do you refer private-duty or non-medical home care?

Our home health category is for skilled, doctor-ordered care. If you also offer private-duty services, tell us in your application and we will note it for families who need both.

Can we limit referrals to certain ZIP codes?

Yes. List exactly where you can staff visits, and update it any time your coverage changes.

In an emergency, call 911. For a mental health or substance use crisis, call or text 988 (Suicide & Crisis Lifeline), any time.
