Who we send your way

The families we connect with assisted living partners

  • Parents who need help with bathing, dressing and medications
  • Older adults who are safe but lonely and isolated at home
  • Caregivers who have reached their limit after months of daily help
  • Families looking for a community that accepts a Medicaid waiver or VA benefits
  • Couples who want to stay together when only one needs care

Families always choose their own provider. We introduce options; we don't make the decision for them.

How the partnership works

1

Apply

Tell us about your organization, locations, licenses and the payers you accept.

2

Standards review

We verify licensing and certification and review the items on this page before any family is referred. [Describe review criteria]

3

Partnership call

We walk through how introductions work and the terms of the partnership. [Partnership terms pending attorney review]

4

Introductions

When a family who fits your services and service area asks to be connected, we introduce you with the details they agreed to share.

Assisted Living partners: common questions

Do you pre-screen families?

We learn about care needs, health, budget and timing before an introduction. Your own assessment still decides whether a resident is a good fit.

Do you refer Medicaid waiver residents?

Only to communities that tell us they participate. If you don't, we won't send you families who depend on a waiver.

Can we pause referrals when we are full?

Yes. Let us know when availability changes and we will adjust who we send your way.

In an emergency, call 911. For a mental health or substance use crisis, call or text 988 (Suicide & Crisis Lifeline), any time.
