Who we send your way

The families we connect with hospice partners

  • Adult children whose parent's doctor has just raised the word "hospice"
  • Spouses caring at home who need after-hours support and equipment
  • Families of assisted living and nursing home residents who want hospice brought to the room
  • Families leaving the hospital after a decision to stop curative treatment
  • Caregivers comparing hospices after a poor experience with a first agency

Families always choose their own provider. We introduce options; we don't make the decision for them.

Hospice partners: common questions

Do you refer only Medicare patients?

No. We work with families on Medicare, Medicare Advantage, Medicaid, private insurance and the VA. Your application tells us which payers you accept so we only send families who fit.

Will families be steered to one hospice?

No. Families choose their provider. Our job is to explain the options that serve their address and fit their needs, and to make the introduction they ask for.

Do you work with facility-based hospice teams?

Yes. Many of the families we hear from have a loved one in assisted living or a nursing home. Tell us which facilities you already serve.

In an emergency, call 911. For a mental health or substance use crisis, call or text 988 (Suicide & Crisis Lifeline), any time.
